Professor Deniz Perin-Coombs uses this Getting to Know You Questionnaire at the start of the semester in her FYW courses. You can download a copy of the the document here.

This questionnaire can be a helpful tool for professors to learn more about each of their students as both a person and a learner. Professor Perin-Coombs notes, “The more I know about you, the better I can support you.”

Included in the questionnaire is also a question about consent to use student work in the classroom, and space for students to list any questions they may have about the course or syllabus.
